What is Continuing Health Care?

Continuing Health Care is a package of continuing care that is arranged and funded solely by the NHS.

Continuing Care is provided over an extended period of time to an individual to meet their physical or mental health needs that have arisen as a result of disability, accident or illness.

Continuing Health Care places no limits on the setting in which the package of support can be offered and can therefore be provided to those in a care home or even their own home.

Who is eligible for CHC?

If an individual’s ‘primary need’ is for health, they will be entitled to Continuing Health Care and their care fees will be fully funded by the NHS.

Eligibility is not dependent on a particular disease, diagnosis or condition.

In order to determine if an individual is eligible for Continuing Health Care, a comprehensive assessment of their health needs is required where health services might be needed. An assessment will therefore be required when an individual is either discharged from hospital or where there is a significant deterioration in the physical or mental health of the individual.

The assessments of needs should involve all healthcare professionals (Multi-Disciplinary Team) involved in the care. The family of the individual should also be involved in the assessment process.

The Multi-Disciplinary Team should look at all of the individual’s health needs before making a recommendation on eligibility. 

Eligibility for Continuing Health Care is not means tested.
